Our Direct Primary Care (DPC) membership gives you a simple monthly subscription with 24/7 access to your doctor: unlimited virtual care by phone/text/email, in-clinic visits without copays, discounted labs, wholesale medications, and much more. Many members describe it as affordable “concierge” care; to us it’s simply the kind of common-sense medicine you’d expect from a community that looks out for one another.

Per Individual
Under 20 years:
With enrolled adult parent = $20
With no adult parent = $40
Ages 19-30 = $60
Ages 31-44 = $60
Ages 45-64 = $85
Ages 65 & up = $100
Enrollment fee= one-time $50 fee per new billing account (family/household)

Personal provider communication (text message, email, phone) = included
Clinic visits (regular hours) = included
DOT Physical = included ($100-200 value!)
Testing
EKG = included
Dipstick Urinalysis = included
Rapid Strep Testing = included
Urine Pregnancy Test = included
Outpatient Procedures (included or deeply discounted!)
Joint injections = no fee
Skin procedures - no fee
Cryotherapy = $10
PRP or PRF Treatments = $200 for one site, +$50 for each additional
Circumcision = $75
Ear Piercing = $30-50
Specialist referral and management = included
After-hours clinic visit = included for urgent matters at discretion of your clinician
House call (regular hours) = included and available at discretion of your clinician. (Transportation fee may apply if not within a 10-15 minute drive from our clinic.)
Discount pricing on labs, radiology (x-rays, scans, etc.), prescription meds, supplements.
